Jennie Cortese, RN | Executive Director, AssistedCare & Eldercare at Home

Jennie Cortese is a Registered Nurse who brings over 40 years of healthcare experience to her role as the Executive Director of AssistedCare & ElderCare. With an extensive background in home care, leadership and executive management, Jennie has overseen multiple Licensed Home Care Branches in various states. She is well-versed in Private Pay, Medicaid, Managed Care, Personal Care Aide Services and Private Duty Nursing.

Jennie’s accomplishments include successfully leading multiple sites through the accreditation process with Community Health Accreditation Partner (CHAP) and the Joint Commission of Accreditation, with commendation, earning recognition for exceptional training programs and community service. She has developed and led initiatives to enhance patient outcomes and reduce rehospitalizations, demonstrating her commitment to continuous improvement in patient care.

Actively involved in professional organizations, Jennie contributes her expertise to the Home Care Forum’s Mastermind Committee and has held positions within the NYS Association of HealthCare Providers, advocating for home care services at both state and federal levels.

In her current role, Jennie finds fulfillment in working with a dedicated team and serving a rapidly growing community. Mentoring new staff and helping clients and their families create customized care plans bring her much satisfaction. Jennie is very passionate about home care and the ability to be instrumental in keeping people home, where they most want to be. Outside of work, she enjoys gardening, boating, and spending time at the beach. Jennie is a music enthusiast and looks forward to playing more golf in the future.

Jennie is a proud mother of two and a first-time grandmother to a grandson in Florida.

Sterling Pope, RN, BSN, QP | Clinical Program Manager

Sterling Pope, RN, BSN, QP, is a highly accomplished healthcare professional with a diverse background and over 10 years of experience in the industry. She currently serves as a Clinical Program Manager, overseeing multiple programs and leading teams to provide exceptional care to individuals in the comfort of their own homes. Sterling’s educational background includes a Bachelor of Science in Nursing from the University of North Carolina Wilmington (UNCW) and the esteemed qualification of Qualified Professional (QP) in the I/DD community.

Sterling’s journey in the healthcare industry began with AssistedCare, where she started as a Certified Nursing Assistant (CNA) just two days before her 20th birthday. This opportunity allowed her to work while pursuing her degree, making it a remarkable birthday gift. After successfully passing her boards, Sterling was promoted to the role of “RN Visit Nurse,” where she supervised CNAs in her previous position.

In recognition of her exceptional skills and dedication, Sterling was promoted to run AssistedCare’s Private Duty Nursing program. This role enabled her to elevate the level of care by transitioning her field staff from a team of aides to a team of fellow nurses. Her contributions were further recognized in 2018 when she assumed leadership of not only the Private Duty Nursing team but also the Empowered (I/DD) program. This experience broadened her understanding of the industry and allowed her to positively impact the lives of diverse populations.

Currently, as a Clinical Program Manager, Sterling continues to excel in her field, managing various programs and teams while expanding her knowledge in financial management, growth strategies, and regulatory compliance within the homecare sector. Her journey of personal and professional growth over the past decade has instilled in her a deep sense of gratitude and motivation to continue learning and making a difference in the lives of those he serves.

Beyond her professional endeavors, Sterling shares a fulfilling life with her middle school sweetheart, Michael, and their beloved chihuahua, Rico. She has cultivated a sense of adventure, having traveled to nine countries and counting. Sterling also finds solace in the tranquility of the beach, indulges in reading, tends to her vegetable garden, and revels in engaging in various DIY projects.

Lexi Weldon, MSN, BSN, RN | Home Care RN Liason

Born and raised in Wilmington, NC, Lexi Weldon brings nearly a decade of nursing experience to her role as a Home Care RN Liaison. After earning her BSN from East Carolina University, she started her nursing career at Duke University Hospital in the Neonatal ICU. Lexi then relocated to NHRMC, where she gained valuable experience in the Operating Room, completing a six-month Perioperative 101 training program. She later transitioned to Wilmington Plastic Surgery as a Clinical RN, balancing her full-time role while completing her Master’s in Nursing Education at the University of North Carolina Wilmington.

Lexi’s dedication to patient care stems from her deep family connections, particularly her grandmother, who inspired her to pursue nursing. She is passionate about treating patients with the same care and compassion she would want for her own family. Outside of work, Lexi is a proud member of the Sigma Theta Tau International Honor Society of Nursing and enjoys boating, walking the loop, and spending time with her family—her husband Sam, their son Everette, and their black lab Goose.

Rebekah Krohn | Recruiter/Service Coordinator

Rebekah Krohn is a dedicated professional in the field of geriatrics, currently serving as a Recruiter/Service Coordinator. Born in Springville, NY, Rebekah pursued her education at Stockton University in Galloway, NJ where she obtained a Bachelor’s degree in Social Work with a minor in Gerontology in May of 2022.

During her academic journey, Rebekah interned at a Long Term Care facility, gaining valuable experience and knowledge in supporting and advocating for the geriatric population throughout their late-life stages. Inspired by personal experiences, particularly her grandmother’s battle with ALS, Rebekah developed a deep passion for making a positive impact on the lives of older adults.

Rebekah’s commitment to enhancing the quality of life for seniors led her to achieve certification in creative storytelling for seniors living with Dementia through TimeSlips. This initiative empowers individuals to express themselves and fosters meaningful connections, revolutionizing the perception and experience of aging.

In her current role, Rebekah finds great satisfaction in identifying the perfect candidates to meet clients’ needs and establishing strong working relationships with caregivers, fostering a sense of community within the team. With a focus on the homecare industry, Rebekah is excited to embark on a fulfilling career dedicated to the well-being of older adults and the I/DD community.

Beyond her professional endeavors, Rebekah enjoys various hobbies and interests, including traveling, nurturing her plant collection, attending concerts, going to the beach and even cliff jumping.

Breona Warchol | Service Coordinator

Born in Florida, Breona Warchol is currently working toward her business degree, with a focus on medical business. She has a strong passion for healthcare and helping others, which led her to pursue a career as a Service Coordinator. Prior to this role, Breona worked as a Patient Support Specialist at Central Vermont Medical Center (CVMC), where she provided support to adult oncology patients. Her two years in the industry have been driven by a desire to create a welcoming and supportive environment for those in need.

Breona enjoys the opportunity to meet new people and thrives in office settings, where she can contribute to a positive workplace culture. Outside of work, she loves exploring beaches, traveling, and trying new restaurants, especially in North Carolina. A devoted pet parent, she also enjoys spending much of her free time with her three cats.
